Top bullet as a short list of information. "...when: Renewing a license, upgrading a license, transferring a license from another state or out of necessity due to changes in job requirements."
Bottom bullet references the top. "Note: The above [is/are] the minimum, federally-regulated conditions in which..."
I really want to think it's the singular "is" but because the "above" reference items in plurality...beyond the fact that I could just re-word this, what's the right answer to this, ya think...?

"The above" would be considered singular even though it's referring to a plural, I believe. The "is" would be referring to "the above", not the plural statements.
Had you said "The above statements", then you would say "are". But I'm fairly certain in this context, "is" is the proper word.

Yeah, I think it's "are" now...because remove the adjectives. "The above are the conditions..." or "The above is the conditions..." or replace "the above" with all of the conditions themselves and it would be "are"...

Now I'm second-guessing myself again. It'd definitely be "the above conditions are," obviously. But when you re-arrange it, "conditions" becomes almost a modifier for "the above," instead of just being the subject, i.e. "The above conditions are," vs. "The above [content] is the conditions in which."
So now I think it may be "is" again
Let me put it this way: whichever one is actually correct, "are" sounds more natural in that case, even if it's not, so nobody is gonna know if that's what you go with.

I think is/are refers to the word "conditions", so you'll want to use "are". That's not even looking at the top bullet.
